What We Do ...
Assessment

Once an individual moves in to the accommodation offered there will be a 12 week assessment period. This will establish what level of support is required by the individual and what support is needed by outside agencies. At this point support plans will be completed by a Key Worker / Link Worker and the individual. These plans are a method of ensuring the individual receives the correct level of support and that all staff are consistent in their approach and support to the individual.
Location

The Lyme Trust has seven houses which are close to the town centre:
  • 33 London Road is a male only residence which offers full board facilities, 9 Single bed spaces
  • 37 London Road is a mixed residence Which offers independent living
  • 39 London Road is a males only residence which offers self catering facilities, 7 Single bed spaces
  • 41 London Road is a male only residence which offers full board facilities, 9 Single bed spaces
  • 52 London Road is a male only residence which offers self catering facilities, 4 Single bed spaces with shared facilities
  • 23 Well Street is a male only residence which offers self catering facilities, 3 single bed spaces
  • The Peter Evans House, 4 Grosvenor Road is a male only residence which offers a resettlement programme, 6 single bed spaces
The Lyme Trust is short term supported housing accommodation for people with mild to moderate health needs whilst promoting independence.
Self-Catering
 
This comprises of a minimal level of support and meals are not included. Staff are there to support the service user in the planning, purchasing and cooking of food for meals. They also provide minimal support in maintaining independent living skills so that  the service user can move onto a more independent living style.

Full Board Facilities
 
This is the standard of accommodation that is offered to new service users who require more support with their daily living skills. This comprises of 3 meals per day, catering for specialized diets e.g Vegetarian, Diabetic etc. We offer the required level of support for service users to maintain a good standard of cleanliness within their bedrooms.
The Resettlement Programme

The ethos of The Lyme Trust resettlement programme is to support individuals to go onto successful independent living.  This will be achieved by empowering the individual with the knowledge and understanding to enable them to make informed decisions.
  • Practical tasks around the home
  • Cooking and Health & Safety Issues
  • Effective Budgeting Support
  • Support in applying for own tenancy
  • Advice and Guidance in accessing voluntary activities
  • Work towards gaining paid employment
  • Social and Leisure activities
Every resident will have a support package tailored to their individual needs which will be reviewed on a quarterly basis.

Residents will feel confident that they have the appropriate support, advice and information available to enable them to move forward in their lives and back to independent living in the community.

The aim of the group is "To promote Independence and Improve the Quality of Life"

In order to achieve its aim the group will:
  • Promote opportunities that improve health and wellbeing
  • Organise meetings, training opportunities and events
  • Publicise and promote work
  • Raise funds
  • Work with similar groups and exchange information and advice
Membership of the group is open to any individual and meets on a monthly basis
